Important Network Information
Progressive Dental Network is NOT A DENTAL INSURANCE. We do not pay claims. All charges for dental services are to be paid by the member directly to the dentist. It is at the dentist’s discretion whether fees for service are due on the spot or in installments. All listed procedures are offered at reduced rates and must be performed by participating dentists.
The fees shown for procedures are reduced fees for usual and customary services. Please discuss all fees with the dentist prior to treatment. Any treatment that requires any unusual or ancillary service may be subject to additional charges including, but not limited to, the following:
· any prosthetic appliance that requires characterized dentures, etc.; · any orthodontic treatment that requires surgery, maxillary expansion, or headgear; · any periodontal treatment that requires root resection or gingival grafts; · any impacted tooth or molar RCT that presents unusual difficulties and circumstances.
It is at the dentist's discretion whether or not to charge for missed appointments, providing copies of x-rays to members, or a per office visit surcharge for sterilization procedures.
Any member accepted for orthodontic treatment must remain a member of the Network for the full duration of their treatment or may be subject to additional charges from their participating orthodontist.
Orthodontists may charge an additional fee for extremely complex or lengthy cases (longer than 24 months).
We cannot guarantee the continued participation of any dentist. It is the members' responsibility to confirm when calling for an appointment that the dentist is currently participating in the Network. Should your dentist end participation the Network, you will need to select another participating dentist. Work in progress after enrollment in the Network must be completed before selecting another participating dentist.
Specialty Services In some areas you may receive a reduction from standard specialty fees from participating specialists, although under the terms of the Progressive Dental Network, there is no specific coverage for specialty care. The Progressive Dental Network makes no claim that all categories of specialty care are available or conveniently located. Call the Member Services Department for the names and locations of participating specialists. |

Length of Membership
Signing Up
Before signing up for the PDN, we suggest you read our Disclaimer and make sure there is a PDN Participating Provider in your area. You should also check the Membership Plans section to determined rates for the type of membership desired. You may then enroll online with a check or by mail.
Cancellation of Membership
You may cancel your membership in writing, when annual renewal is due. We reserve the right to cancel any member for lack of payment sixty days after payment is due.
Signing up after Membership Cancellation
In order to prevent members from canceling and signing up again when Dental Care is needed, all members who have canceled in any form and would like to sign up again, will be required to pay all membership fees from the date of cancellation or two years worth of membership fees, which ever is less. |

Disclaimer This plan may not cover all procedures which might be provided by your dentist. Any procedure not listed on the Network Fee Schedule may cause additional costs to be incurred. Please be aware that the dollar amount specified for each procedure may not be the only cost incurred for a given treatment because the treatment may require more than one dental procedure. Your dental services will be available and will be provided at such times as deemed necessary and practical by the participating dentist. Your dental services will be in accordance with accepted dental practices and standards prevailing at the time of treatment in the area of service coverage and in conformity with the professional and technical standards adopted by The Dental Review Board. Operative procedures recommended by a participating dentist are to be performed at the time mutually agreeable to the subscribing member and to the participating dentist.
Membership in the Progressive Dental Network may be terminated for non-payment of your membership plan fee, misuse of the ID card or non-payment of dentist’s fees. |
