There are Two Types of Network Providers |

Associated Providers list their own percentage of discounts that they are willing to extend to our members.
* Minor Procedures are procedures that cost less than $1,000. **Major Procedures are procedures that cost more than $1,000 of the treating provider’s usual charges for the same services.
A single treatment that requires several minor procedures totaling more than $1,000 will be considered and billed at the major procedure discount level.
Please note that Associated Provider’s fees may vary. Always inquire before starting treatment.
Out of Area Services are available out of area only through another network provider. Please refer to our list of Participating Providers on this website or contact Member Services for the provider nearest you. |
