We offer two ways of becoming a Participating Provider
Preferred Network Provider: As a Preferred Provider, you will be offering dental services to our members at reduced rates as indicated in the PDN Fee Schedule for your area.
Associate Network Provider: You may join PDN as an Associate Provider offering members the following discounts:
* Minor procedures treatment that totals less than $1000 **Major procedures treatment that totals more than $1000

20% discount on *Minor procedures 30% discount on **Major procedures |

A single treatment that requires several minor procedures totaling more than $1,000 shall be considered and billed at the Major Procedure discount level. |
